Important Note: Passports are processed by the U.S. Department of State. The Office of the City Clerk performs in-person application review and acceptance before forwarding your documents to the U.S. Department of State for processing. You do not need to be a Henderson resident to use this service.

The Office of the City Clerk is experienced with complex applications (e.g., delayed birth, birth abroad). Please complete and print (single sided only) a Passport Application (Form DS-11) prior to your visit. Filling the form out by hand? Please use a black ink pen.

BEWARE OF COMPANIES CLAIMING THEY CAN EXPEDITE YOUR PASSPORT OR CHARGING FOR AN APPLICATION. These may be scams and may put your information at risk. Customers traveling in 14 days or less can use an authorized hand carry company. Please research the requirements for hand carry online as we cannot endorse or recommend a specific company. 

(Adult renewals of passports which are current or expired less than 5 years may be renewed by mail without an agent on form DS-82, please click HERE to see if you qualify and to get instructions. The City of Henderson does not process DS-82 forms.)

1. Both parents and child must be present to process application 
   • One parent and the child may appear and present documentation of sole legal guardianship
   • One parent and the child may appear with an original Statement of Consent (Form DS-3053) signed by the absent parent and notarized, as well as a copy of the absent parent’s valid ID front and back.   

2. Proof of US citizenship
   • Birth certificate showing birth in the United States, (must include full name of parent(s)); or
   • Certificate of Naturalization (Form N-550); or
   • Certificate of Citizenship (Form N-560); or
   • Form FS-240, Report of Birth Abroad of United States Citizen; or
   • Valid U.S. passport.

3. Valid photo ID of both parents AND proof of parental relationship (original documents) includes birth certificate, court order, adoption papers, divorce/custody decree, etc. Note: Foreign birth certificates not in English must be accompanied by an official translation.

**Passport cards are only valid if you plan to travel into the U.S. by land or sea from Canada, Mexico the Caribbean, and Bermuda. If you plan to fly, you must obtain a Passport Book for reentry to the US.

There is a $35.00 processing fee per application, payable to the City of Henderson. This fee can be paid as card, cash, check, or money order.

In addition, you can have passport photos taken at our office for $12.00 per application. This fee can be paid as card, cash, check, or money order.

DO NOT combine the City of Henderson fees with your U.S. Dept. of State payment or the PAYMENT WILL BE REJECTED.

 ***The City of Henderson will charge a 2.8% nonrefundable service fee for most credit or debit card payments, beginning March 1, 2024 (some exclusions may apply). To continue providing convenient options and alleviate any inconvenience with the card service fees, the following free options may be available (where applicable):

• Cash payment in person
• Check or money order payment in person

The U.S. passport card facilitates entry and expedites document processing at U.S. land and seaports-of-entry when arriving from Canada, Mexico, the Caribbean and Bermuda. The card may not be used to travel by air. It will otherwise carry the rights and privileges of the U.S. passport book and will be adjudicated to the exact same standards.        

A passport book is an internationally recognized travel document that verifies the identity and nationality of the bearer. A valid U.S. passport is required to enter and leave most foreign countries. Only the U.S. Department of State has the authority to grant, issue, or verify United States Passports.

• Pupils and irises must be clearly visible

• Full face must be visible and free of shadows

• Neutral facial expression with both eyes open and mouth closed

• Hair must not obscure the face

• Glasses must be removed even if glasses are routinely worn

• Uniforms, such as military or police, must not be worn

• Camouflage clothing should not be worn

• Religious clothing worn daily may be worn, but must not obscure the face

• No hats or head coverings, except for religious purposes. A signed statement regarding religious headgear must be requested.

• Photo must not show headphones or a wireless device, such us Bluetooth headset or earbuds

• Photo must not be retouched or contain a filter

We are in City of Henderson West City Hall at 240 S. Water St., Henderson NV, 89015, in the Development Services Center. The line forms at the Basic Road entrance to City Hall, not at the front Water Street entrance.

Visitor parking is available in the parking garage at Basic Rd./Lead St. and street parking is available on Basic Road.
